Admission Checklist

AS in Nursing Checklist:

_____ I am 18 years of age with a valid driver’s license.
_____ I have a high school diploma or G.E.D. or official college transcript.
_____ I have completed the Enrollment Agreement and Application with appropriate fees.
_____ I have a current AHA BLS for healthcare provider card.
_____ I received a physical signed by a physician and performed within the last 12 months.
_____ I have taken a current 5 panel drug screen (current within 6 months).
•    If you are a healthcare provider such as a current Florida licensed paramedic- Copy of paramedic license.

Emergency Medical Services Student File Checklist:

_____ I have a signed Student Application.
_____ I have a signed Student Enrollment Application.
_____ I have a current Florida driver’s license/ID.
_____ I have a high school diploma or G.E.D.

In addition to above:
EMS (EMT and Paramedic) Admission Requirements:
_____I have an EMT license/PM ONLY EMT Expiration Date_____.
_____I have completed the FDLE Consent Form.

Paramedic Admission Checklist:
_____ I have and can present a valid Florida State EMT license or be certified by Phase I of the program.
_____ I have and can present a high school diploma or G.E.D.
_____ I am at least 18 years of age.
_____ I have a physical/health clearance from a physician (Health Forms provided).
_____ I have received a 5 panel drug screen.
_____ I have proof of health insurance (recommended).
_____ I have a valid driver’s license or government issued photo ID.
_____ I have submitted payment for the first semester’s tuition and fees.

AS EMS Admission Requirements Checklist:
_____ I meet all HCI general entrance requirements.
_____ If I am not a Certified Florida State Emergency Medical Technician, then I must follow the criteria    presented for EMT registration requirements.
_____ If I am a Certified Florida State EMT, then I must follow the criteria presented for Paramedic registration requirements.
_____ If I have completed either or both EMT and Paramedic programs with Health Career Institute before enrolling in the AS program, I am encouraged to consult with an advisor so as to obtain credit(s) towards my degree.
